Temporary Financial Assistance Program (TFAP)
The MTS Sickle Cell Foundation is proud to announce the AccessCard Temporary Financial Assistance Program. Through this program, transportation and nutrition assistance will be provided to families impacted by SCD.
**As a non-profit organization, we rely on the generosity of our sponsors. Program continuation is contingent on the availability of funds. The program could be modified or discontinued at any time if funding is limited or no longer available.**
ELIGIBILITY REQUIREMENTS
- Patients/Caregivers receiving assistance must be Georgia residents with a valid Georgia Driver's License.
- Patients must have a confirmed Sickle Cell Disease diagnosis, be in active treatment, scheduled to begin treatment or in follow up care, all attested by the person completing this form.
- Patient/Caregiver must have No/Limited Income, Unexpected Admission/Emergency Department Visit, and or Unable to access immediate external/community resources.
